Information technology and information management are key elements that enable the empowerment of patients, and connect their continuum of care. They eliminate the need to capture the same information from Central West LHIN residents multiple times and support patients, their caregivers, and their health service providers to share information and coordinate services quickly and efficiently as they transition from one care provider to another.
Leveraging information management and information technology investments made at the local, regional, and provincial levels will drive health care transformation and enable informed system planning decisions. Solutions have been identified for implementation over the next three years will support the IHSP and are identified in the Enabling Technologies for Integration (ETI) PMO Funding Business Plan–Central Ontario Cluster–including eConsult, eNotifications, Connecting GTA (cGTA), Hospital Report Manager (HRM), eReferral, Electronic Medical Record (EMR) and the Community Services Information Management/Information Technology (IM/IT) Model.
The Central West LHIN will work in partnership with the Ontario Telemedicine Network (OTN) to leverage the effective and appropriate use of telemedicine technologies (e.g. Telehomecare and Personal Computer and Video Conferencing) to improve access to care for Central West LHIN residents. These OTN tools will be implemented in established clinical pathways and health care services that will add value and ensure patients and providers have an enhanced experience through care delivery.
