Defining Success

As the Central West LHIN implements its strategic plan it will define the following as success for the region’s health care system. A local health care system that is:

  • accountable to funders and to the community, and that demonstrates transparent decision-making
  • focused on delivering high quality services to the patient that achieves desired patient and health care outcomes
  • easy to access and navigate
  • engaging patients, caregivers and providers to understand and respond to their unique needs in the community
  • collaborative and innovative to deliver seamless care to the community
  • based on a population health perspective
